If you have an upcoming appointment, you will be provided with a Security Code to access the Patient Portal.  Please use the Patient Portal link to complete your Medical History questionnaire or paperwork that pertains to you and your appointment.  You can access existing information that you have on file with us through the portal, as well.

Monday: 8:00am – 4:30pm (Lunch between 12:30 – 1:30)
Tuesday: 8:00am – 4:30pm (Lunch between 12:30 – 1:30)
Wednesday: 8:00am – 4:30pm (Lunch between 12:30 – 1:30)
Thursday: 8:00am – 4:30pm (Lunch between 12:30 – 1:30)
Friday: 8:00am – 3:00pm (Lunch between 12:30 – 1:00)

We accept Cash, Check, Credit Cards (MasterCard, Discover, Visa, American Express), and CareCredit® credit card.


CANCELLATION POLICY & Missed Appointments

Due to the increased volume of patients, we have had to develop a more stringent Cancellation & Late Policy. We realize that circumstances may cause you to arrive late or miss an appointment.

Existing patients and clients should arrive 5 minutes prior to scheduled treatment time to check in.

Should you need to cancel, please contact us within 48-business hours prior to your scheduled appointment. By doing so, you make it possible for us to see another patient who might otherwise have to wait to be seen.

  • ALL CANCELLATIONS WITH LESS THAN 48-BUSINESS HOURS NOTICE WILL RESULT IN A $25.00 FEE.

  • ALL MISSED APPOINTMENTS WILL RESULT IN A $25.00 FEE WITH THE EXCEPTION OF ULTRASOUNDS A $50.00 FEE.

If you have 2 or more missed appointments or fail to cancel within 48-business hours:

  • You may be charged the full amount of your appointment / treatment

  • If you purchased a package, a treatment may be deducted off of your purchased package

  • Discharged from the practice

The above courtesies enable us to compensate our employees for their time, to help cover costs of opened supplies / pre-drawn medications, and maintain a higher availability of our time for you as well as others. By signing in and/or scheduling an appointment, you are agreeing to our cancellation policy.


LATE FOR AN APPOINTMENT

  • Please call if you know you are going to be late, so that we may try to rearrange our schedule to accommodate you or if necessary reschedule your appointment.

  • Should you arrive 10-minutes late, you may be asked to reschedule.

  • Late arrivals of less than 10-minutes may result in a shortened appointment / treatment time.

Please be aware that if you have 2 or more late arrivals that cause us to reschedule your appointment you may be:

  • Charged a fee of $25.00

  • Charged the full amount of your appointment / treatment

  • If you purchased a package, a treatment may be deducted off of your purchased package
